What is included with the 3DS Capture Board?

I was wondering what was included with the installation of the 3DS Capture Board (sending the 3DS over to have the board installed). Does everything (the USB cable and audio cable/cord thing) come with the installation, or do they have to be purchased separately?

The ground loop isolator plugs into your 3DS and gets rid of static the board produces while in use. Something like this will do for an audio cable, most stores that have an electronics section sell them.
